Two more Trusts share information on the Great North Care Record

North Tees & Hartlepool NHS Foundation Trust and County Durham & Darlington NHS Foundation Trust are now sharing patient information as part of the Great North Care Record programme.

The Great North Care Record electronically connects patient information from GPs, local hospitals, social care, community and mental health teams together across the system, helping to make care better and safer. It operates in the North East of England and North Cumbria and covers the 3.6m people living in our region.

Both Trusts are now making information recorded about patients available electronically as part of the Great North Care Record, which securely shares information such as medications, hospital visits, referrals and letters. This means that health and care staff from other organisations in the North East and North Cumbria can then view this information, helping to make care better and safer.

Organisations already sharing:
• All 400 + GP practices in the North East and North Cumbria
• Over 200 community services
• Newcastle upon Tyne Hospitals NHS Foundation Trust
• Cumbria, Northumberland Tyne and Wear NHS Foundation Trust
• South Tyneside and Sunderland NHS Foundation Trust
• North East Ambulance Service NHS Foundation Trust
• Gateshead Health NHS Foundation Trust
• Sunderland City Council

The aspiration is that patient information will be available from all primary care practices, NHS Trusts and local authorities across the North East and North Cumbria Integrated Care System.
