Durham County Council connects health and social care data

Durham County Council has joined the Great North Care Record to enable its adult social care services team to view information from the NHS.

The Great North Care Record electronically connects the pieces of patient information from GPs, local hospitals, social care, community and mental health teams together across the system. It operates in the North East of England and North Cumbria and covers the 3.6m people living in our region.

This means that health and care staff from other organisations in the North East and North Cumbria who are involved in your care can then access this information, helping to make your care better and safer.

Durham County Council is the latest organisation to view data on the Great North Care Record and is working towards sharing data over the coming weeks, they join:

· All 400 + GP practices in the North East and North Cumbria

· Over 200 community services

· Newcastle upon Tyne Hospitals NHS Foundation Trust

· Cumbria, Northumberland Tyne and Wear NHS Foundation Trust

· South Tyneside and Sunderland NHS Foundation Trust

· North East Ambulance Service NHS Foundation Trust

· Gateshead Health NHS Foundation Trust

· North Tees and Hartlepool NHS Foundation Trust

· County Durham and Darlington NHS Foundation Trust

· Sunderland City Council

Jane Robinson, Director for Adult and Health Services, said: “Within County Durham we are committed to providing the best care we can for people across health and social care agencies. For us, the integration of the Great North Care Record viewer within our case management system is an important step forward, enabling social workers and other care professionals to have a quick and easy view of key critical information held in health systems. This will enable us to further improve the support provided by social care.”
